A groundbreaking study published in Nature Materials has uncovered a promising new strategy to combat heart aging, with scientists suggesting that targeting the extracellular matrix (ECM) may help slow — or even reverse — age-related cardiac decline.
As we age, the heart — like other organs — loses its flexibility and efficiency. Much of this degeneration has traditionally been attributed to cellular aging. But now, researchers at the National University of Singapore have shifted focus to the extracellular matrix — the biological scaffold that surrounds and supports cells.
The extracellular matrix is a complex network of proteins and molecules between cells. It helps with tissue structure, communication, and repair. As the ECM stiffens with age, it can trigger cardiac fibrosis — the buildup of connective tissue — which hinders the heart’s ability to pump effectively.
This stiffening process activates cardiac fibroblasts, which in turn accelerate aging of the heart tissue. While fibrosis aids healing after damage, persistent and uncontrolled fibrosis contributes to heart failure over time.
To better understand ECM’s role, the team developed DECIPHER — a novel platform combining real rat heart tissue and lab-engineered hydrogels. DECIPHER allows scientists to independently test the effects of mechanical stiffness and biochemical signaling on heart cells.
Lead researcher Avery Rui Sun explained:
The results were compelling:
This study implies that the biochemical environment, not just physical stiffness, plays a major role in aging. It could pave the way for therapies that target the extracellular matrix rather than just cells — offering potential for reversing cardiac aging.
According to Dr. Nishant Kalra, interventional cardiologist and chief medical officer at VitalSolution, who was not involved in the study:
While promising, these findings are still in preclinical stages and based on rat models. Translation to human therapy will require further validation.
Until then, experts recommend proven strategies to support heart health as you age:
✅ Regular aerobic exercise: At least 150 minutes of moderate or 75 minutes of vigorous activity weekly
✅ Heart-healthy diet: Prioritize vegetables, fruits, whole grains, lean protein; limit saturated fats and sodium
✅ Quit smoking: Smoking accelerates vascular and cardiac aging
✅ Adequate sleep: 7–9 hours per night
✅ Weight management: Maintain a healthy BMI
✅ Manage stress: Use mindfulness, exercise, or nature to reduce cortisol
✅ Limit alcohol: Keep under 14 units/week and allow breaks from drinking
Dr. Daniel Atkinson, a UK-based physician, also emphasized cutting down on alcohol:
