The future of blood pressure monitoring is evolving — cuffless devices promise convenience and comfort, though traditional monitors remain essential for now. The FDA has cleared the first over-the-counter cuffless blood pressure device, known commercially as the Hilo Band (G0 Blood Pressure Monitoring System).
As hypertension continues to affect over 1.3 billion people globally, innovations that lower barriers to frequent measurement could improve detection and management. The cuffless device is not yet a replacement, but it signals a meaningful shift toward seamless, wearable health tech.
The Hilo Band is a slender wristband that uses photoplethysmography (PPG) — a light-based sensor technique — to detect changes in blood flow through vessels of the inner wrist. It can take multiple readings per day (up to 50) and works continuously day and night.
In July 2025, the FDA granted 510(k) over-the-counter clearance for the Hilo Band to be used by adults at home. That makes it the first cuffless blood pressure monitor approved for OTC use in the U.S. Clinical trials compared it against standards like ambulatory blood pressure monitoring, home devices, and arterial catheter measurements, and the results supported its accuracy under ISO 81060-2 standards.
The Hilo Band is expected to hit the U.S. market in 2026, priced around US$280. Users will need to calibrate monthly using a traditional cuff (provided) to maintain accuracy.
Beyond wristbands, several emerging approaches aim to measure blood pressure without cuffs. Researchers are exploring rings, smart glasses, chest patches, temporary tattoos, and even toilet seats as possible platforms. Some cuffless devices are already cleared for use in medical settings, though not yet for general consumer use.
These devices rely on advanced signal processing, pulse wave analysis, and calibration algorithms to infer systolic and diastolic pressure from optical signals.
While cuffless devices offer exciting potential, standard validation and regulatory guidelines are still catching up. As Dr. Howard LeWine (Brigham & Women’s Hospital) cautions: we’re not quite there yet. Accuracy under varied conditions—motion, skin tone, temperature, vascular conditions—remains a key challenge.
Regulators and industry bodies are working on uniform standards to evaluate cuffless devices’ performance. Until then, traditional cuffs and ambulatory monitors remain the gold standard, especially for diagnosis or treatment decisions in clinical settings.
