Sucralose consumption linked to reduced cancer immunotherapy response, study warns

A new study published in Cancer Discovery, a journal of the American Association for Cancer Research (AACR), has found that sucralose, one of the most commonly used artificial sweeteners, may reduce the effectiveness of cancer immunotherapy by negatively altering gut microbiota and disrupting essential immune functions.

This research raises critical concerns for cancer patients who regularly consume sucralose through diet sodas, sugar-free foods, and low-calorie beverages, suggesting that their treatment outcomes could be compromised by this non-nutritive sweetener.

How sucralose affects cancer treatment

The study, led by Dr. Diwakar Davar and Dr. Abby Overacre from the University of Pittsburgh and UPMC Hillman Cancer Center, found that high sucralose intake altered the gut microbiome in ways that decreased T-cell function—a key component in immunotherapy success.

In both mouse models and human trials involving melanoma and non-small cell lung cancer (NSCLC) patients, those who consumed more than 0.16 mg/kg/day of sucralose had significantly poorer responses to immunotherapy and lower survival rates compared to those with minimal or no sucralose consumption.

Gut bacteria degrade essential immune nutrient

Researchers discovered that sucralose-fed mice showed:

  • Reduced infiltration of CD8+ T-cells, critical immune cells responsible for attacking cancer.
  • Increased tumor growth.
  • Decreased survival rates during immunotherapy.

The underlying mechanism involved gut bacteria degrading arginine, an amino acid vital for T-cell activation. This degradation impaired the immune response, weakening the body’s ability to fight cancer cells under immunotherapy.

Arginine supplementation restores immune function

In a promising development, researchers were able to reverse the negative impact of sucralose in mice by providing arginine or citrulline supplements, which restored T-cell activity and re-sensitized tumors to immunotherapy.

Dr. Overacre emphasized that while foods like nuts, seeds, poultry, lentils, and fruits contain arginine, a supplemental approach may be necessary for cancer patients to maintain optimal levels during treatment.

Should cancer patients avoid sucralose?

Although the research is still evolving, both researchers and external experts recommend that cancer patients undergoing immunotherapy limit or avoid sucralose.

Dr. Jack Jacoub, an oncologist not involved in the study, said:

“This is high-quality evidence. If my patients ask what they can do to support their treatment, avoiding sucralose is now a clear recommendation.”

Dr. Overacre added that the research gives patients a proactive measure to improve treatment outcomes and potentially increase survival chances—a significant stride in personalized cancer care.

WHO’s position on non-sugar sweeteners

The World Health Organization (WHO) has already discouraged the use of non-sugar sweeteners (NSS) like sucralose for weight control, noting they offer no long-term benefits and no nutritional value.

This new evidence strengthens the case for re-evaluating the use of artificial sweeteners, especially in individuals with serious health conditions such as cancer.

Researchers are now planning prospective clinical trials to explore sucralose’s impact across different cancer types and treatment models, and to test arginine supplementation in human cancer patients receiving immunotherapy.

The study underscores the urgent need for dietary awareness and physician-guided nutritional strategies during cancer treatment, particularly in the era of personalized medicine.
