KARACHI: As 24 to 26 million people with diabetes in Pakistan prepare to observe Ramadan, medical experts are urging them to seek proper guidance and planning to ensure their health is not compromised. While fasting during Ramadan is a religious practice for many, experts warn that failing to consult with healthcare providers could lead to serious health risks, especially for individuals living with diabetes.

Pre-Ramadan medical advice is crucial for safe fasting

Experts at the 12th International Diabetes and Ramadan Conference emphasized that with proper medical advice, most people with diabetes can safely fast. They suggested a pre-Ramadan risk assessment, medication adjustments under medical supervision, and regular monitoring of blood glucose levels to minimize the risk of complications. Regular blood sugar monitoring (3 to 5 times a day) does not break the fast, and fasting should be interrupted immediately if glucose levels fall below 70 mg/dl or rise above 300 mg/dl with symptoms.

Risks of fasting without medical advice

Prof. Javed Akram, President of the Pakistan Society of Internal Medicine, highlighted that about 70-75% of people with Type 2 diabetes and 40% of those with Type 1 diabetes in Pakistan fast during Ramadan. While fasting is encouraged, medical advice should always be sought before embarking on it to ensure safety. Prof. Akram stressed that Islam encourages not putting oneself in harm's way, and when health is involved, medical guidance becomes part of religious guidance.

Guidance for fasting safely during Ramadan

Dr. Saif ul Haq, a senior diabetologist, emphasized the importance of safe fasting education, as many people with diabetes across Pakistan and the Islamic world still opt to fast. Research and clinical guidelines over the past two decades have shown that with proper diet planning, medication, and regular monitoring, risks such as hypoglycemia and hyperglycemia can be largely avoided. He also highlighted the importance of hydration, proper meal choices, and adequate insulin dosing during the fasting period.

Technology’s role in managing diabetes during Ramadan

Dr. Somia Iqtidar introduced the potential benefits of using digital health applications in local languages to monitor blood sugar, remind patients about medication, and help them follow a safe diet during Ramadan. She encouraged diabetics to use AI-powered apps for better management of their condition during the fasting period.

Prof. Feroz Memon, a leading cardiologist, warned that while fasting can have heart health benefits, it must be done in moderation. Similarly, gastroenterologist Prof. Aftab Mohsin urged people to avoid overindulgence during Sehri and Iftar and to maintain a balanced diet for optimal physical and spiritual well-being.
