WASHINGTON: A major step forward in pediatric infectious disease prevention has been announced as US regulators approved an expanded use of Merck’s pneumococcal vaccine, Capvaxive, for high-risk children and teenagers.
The decision by the US Food and Drug Administration (FDA) extends the use of the vaccine to individuals aged 2 to 17 years who are at increased risk of pneumococcal disease due to underlying chronic conditions.
Expanded protection for vulnerable children
According to the approval details, the vaccine can now be given to children and adolescents who have already completed routine pneumococcal vaccination schedules but continue to face elevated risk because of conditions such as heart disease, lung disorders, kidney disease, liver disease, or diabetes.
These underlying illnesses weaken the body’s ability to fight infections, making pneumococcal disease potentially life-threatening even after standard immunization.
What is pneumococcal disease?
Pneumococcal disease is caused by the bacterium Streptococcus pneumoniae, a pathogen responsible for a wide range of serious infections including: Pneumonia, Meningitis, and Bloodstream infections (sepsis).
The bacteria spread through respiratory droplets such as saliva or mucus, making transmission easier in close-contact environments like schools and households.
Young children under five and older adults are generally considered the highest-risk groups, but children with chronic illnesses also face significantly increased vulnerability.
About Merck’s Capvaxive vaccine
Developed by global pharmaceutical company Merck & Co., Capvaxive is a next-generation pneumococcal conjugate vaccine designed to protect against multiple strains of Streptococcus pneumoniae.
The vaccine was initially approved in 2024 for adults, and this new authorization marks its first expanded pediatric use in high-risk populations.
Capvaxive works by targeting a broad range of pneumococcal serotypes, aiming to improve protection in individuals who may not respond fully to earlier-generation vaccines.
Evidence behind the decision
The FDA’s decision is based on late-stage clinical trial data involving 874 participants, where the vaccine demonstrated immune responses comparable to or better than the older PPSV23 pneumococcal vaccine.
Merck reported that side effects observed during trials were generally mild and short-lived, consistent with typical vaccine safety profiles.
