NIH issues influenza H3N2 advisory as cases surge across Pakistan
ISLAMABAD: The National Institute of Health (NIH) has issued an advisory on the prevention and control of seasonal influenza H3N2, as Pakistan continues to witness a sustained rise in influenza-like illness and severe respiratory infections across multiple regions.
According to the NIH advisory, Influenza A (H3N2) subclade K has emerged globally during the 2025–26 influenza season, with a sharp acceleration in cases reported since August 2025. The advisory comes at a time when hospitals across Pakistan have already been reporting increased patient load linked to flu-related complications for several weeks.
Global H3N2 surge preceded Pakistan’s advisory
International surveillance data cited by the NIH shows that South Asia experienced a major influenza surge between May and November, with 66 percent of reported cases identified as H3N2. Significant outbreaks were also documented earlier in the United States, the United Kingdom, Europe, Japan, Australia, and New Zealand, where health agencies had already issued alerts, updated vaccination guidance, and activated public health response mechanisms.

Public health experts note that the global spread and severity of H3N2 had been evident well before its intensified impact in Pakistan, raising questions about the timing of domestic risk communication.
Pakistan sees sharp rise in ILI and SARI cases
In Pakistan, the NIH confirmed a notable increase in Influenza-Like Illness (ILI) and Severe Acute Respiratory Infection (SARI) cases. Official figures show that 340,856 suspected cases have been reported nationwide so far, with 12 percent of tested samples returning positive for H3N2.
Clinicians across major cities report that the virus has placed visible strain on outpatient departments and emergency services over the past one to one-and-a-half months, particularly affecting elderly patients and those with comorbidities.
Advisory arrives amid growing concern over delayed response
While the issuance of the advisory is a necessary step, public health observers point out that the severity, clinical burden, and healthcare pressure caused by H3N2 had already been widely felt before formal guidance was released. Given Pakistan’s participation in global disease surveillance networks and the early warning signals from international outbreaks, experts argue that earlier public advisories and preventive messaging could have reduced transmission and panic.

- Avoiding crowded and poorly ventilated places
- Seeking prompt medical care if flu symptoms worsen

Influenza H3N2 is known for causing more severe disease and higher hospitalization rates, particularly among older adults. Public health specialists warn that delayed advisories not only impact individual health outcomes but also hinder hospitals’ ability to plan staffing, beds, and infection control measures during peak transmission periods.
As Pakistan continues to battle recurrent seasonal outbreaks, experts underline the need for proactive surveillance, transparent communication, and early public engagement to protect lives and prevent unnecessary strain on an already overburdened healthcare system.
