A new study published in the Journal of Translational Medicine has introduced a promising blood test for chronic fatigue syndrome (CFS) — also known as myalgic encephalomyelitis (ME) — that could diagnose the condition with up to 96% accuracy.

Researchers from the University of East Anglia and biotechnology firm Oxford Biodynamics used an innovative 3D genomics technique to identify unique biological signatures linked to ME/CFS, marking a major step toward an objective diagnostic tool for a condition that affects 17–24 million people worldwide.

Understanding ME/CFS: A complex, misunderstood condition

ME/CFS is a chronic neurological disorder that disrupts multiple body systems, causing extreme fatigue, flu-like symptoms, muscle pain, enlarged lymph nodes, brain fog, and unrefreshing sleep.

Diagnosing ME/CFS has long been difficult since no single biomarker exists and symptoms often overlap with other diseases. Patients typically endure years of uncertainty before receiving a confirmed diagnosis.

How the new ME/CFS blood test works

The newly developed test uses EpiSwitch® 3D Genomics technology to study how DNA folds inside human cells — a process that influences which genes are turned “on” or “off.”

“You can think of DNA like an origami folded in specific ways,” explained Dr. Dmitry Pshezhetskiy, lead author and research fellow at Norwich Medical School, UK. “We found a distinctive folding pattern in ME/CFS patients that isn’t seen in healthy individuals. Using this pattern, we can detect the disease from a small blood sample — quickly, non-invasively, and with high precision.”

Study details and key findings

The research analyzed blood samples from 47 patients with severe ME/CFS and 61 healthy individuals. Results showed that the test could identify the condition with 96% accuracy, a remarkable achievement for such a complex disorder.

Dr. Pshezhetskiy noted, “This level of accuracy means doctors can diagnose ME/CFS confidently, rather than relying on years of exclusion-based assessments. It can save patients enormous stress, cost, and time.”

Beyond diagnosis, the study revealed that over 200 chromosomal conformations were altered in ME/CFS patients, many related to immune and inflammatory pathways. This discovery could help identify potential treatment targets and guide personalized therapy in the future.

Expert opinions: Hope with caution

Dr. David Cutler, family medicine physician at Providence Saint John’s Health Center, said the test offers a long-awaited breakthrough for patients whose symptoms were often dismissed or misdiagnosed.

“It would finally give patients validation that their symptoms — fatigue, dizziness, brain fog, headaches, and light sensitivity — have a biological explanation,” he said. However, he cautioned that the study’s small sample size means further validation is essential.

Dr. Swapnil Patel, vice chairman of medicine at Hackensack Meridian Jersey Shore University Medical Center, agreed, noting, “This is an exciting step forward, but large-scale studies are needed to confirm accuracy across diverse populations and to see if it can distinguish ME/CFS from other fatigue-related disorders.”

The road ahead: From diagnosis to targeted treatment

Experts believe the new test could eventually guide tailored therapies by mapping which biological pathways are disrupted in each patient. This could lead to treatments that address the root causes rather than just the symptoms of chronic fatigue.

As research progresses, the ME/CFS blood test represents both a scientific milestone and a symbol of hope for millions seeking recognition and relief from an often misunderstood disease.
