Time-restricted eating without calorie reduction does not improve key metabolic health markers
In a carefully controlled study assessing the popular approach of “intermittent fasting,” researchers found that adopting a shorter eating window alone did not yield measurable improvements in metabolism or heart-health markers. The trial, led by German Institute of Human Nutrition Potsdam‑Rehbruecke (DIfE) and Charité – Universitätsmedizin Berlin, challenges assumptions about time-restricted eating (TRE) when calorie intake remains unchanged.
What the study involved
Researchers enrolled 31 women with overweight or obesity in a randomised crossover design. Each participant undertook two separate two-week dietary phases:
- Early TRE (eTRE): eating window from 8 a.m. to 4 p.m.
- Late TRE (lTRE): eating window from 1 p.m. to 9 p.m.
Both phases maintained nearly identical calorie and nutrient intake (isocaloric).
During each phase, the team conducted:
- Oral glucose tolerance tests and routine blood sampling to assess glucose, lipids, insulin sensitivity, and inflammation.
- Continuous glucose monitoring, controlled physical activity via motion sensors.
- Analysis of participants’ internal circadian clocks using the BodyTime assay in isolated blood cells.
Key findings
- No clinically significant improvements were observed in insulin sensitivity, blood glucose levels, blood lipid profiles or inflammatory markers after either TRE arm.
- However, TRE did shift participants’ circadian rhythms: the late-eating window (lTRE) resulted in an average ~40-minute delay in internal clocks and later bed/wake times compared to the early window.
Researchers conclude that previously observed health benefits of TRE likely stem from unintentional calorie reduction rather than the reduction of the eating window in isolation.

- The study controlled calories strictly, but real-world TRE may lead to spontaneous calorie reductions, which may explain prior positive findings.
- Shifting internal clocks may still have other health implications; the absence of immediate metabolic improvements does not rule out longer-term benefits or benefits via other pathways.

- If using TRE for weight or metabolic health, ensure you monitor energy (calorie) intake — window restriction may not suffice alone.
- Consistent healthy diet quality, calorie control, and physical activity remain central to metabolic health — TRE is not a magic shortcut.
