Can the US achieve cervical cancer herd immunity through HPV vaccination?
The United States is on the brink of a major public health victory — herd immunity against the human papillomavirus (HPV), the leading cause of cervical cancer worldwide. A growing body of research, including a new study published in JAMA Pediatrics, highlights how sustained HPV vaccination efforts are dramatically reducing infections and related cancer rates across the country.
HPV is the world’s most common sexually transmitted infection, responsible for nearly 95% of the 660,000 cervical cancer diagnoses each year. But with vaccines now protecting against nine key HPV strains responsible for most cancers, experts say the US may be approaching community-level protection — a milestone that could eventually prevent thousands of cervical cancer cases annually.
Evidence of herd immunity emerging
Researchers from the Albert Einstein College of Medicine in New York analyzed nearly two decades of HPV data and found striking reductions in infection rates — even among unvaccinated women. The study’s senior author, pediatrician Dr. Jessica Kahn, explained that vaccination of both boys and girls has created strong indirect protection across communities.
“Our study demonstrates evidence for herd immunity against cervical cancer-causing HPV in one community and in the setting of high vaccination rates and a gender-neutral vaccination strategy,” said Dr. Kahn.
Among women at high risk of HPV infection, vaccine-covered strains decreased by 98% with bivalent vaccines. Notably, unvaccinated women in the same community experienced a 75% decline in infection by those strains, suggesting widespread community protection.
However, Dr. Kahn cautioned that herd protection is dynamic and reversible. “There is not a specific fixed point at which we can say that HPV is eliminated,” she said. “Herd protection can wane if vaccine uptake decreases, individuals become more susceptible, or transmission dynamics change.”
The science behind HPV and cervical cancer
HPV viruses were first linked to cervical cancer by German scientist Harald zur Hausen in 1983, a discovery that earned him a Nobel Prize in Medicine in 2008. While most HPV infections are harmless and symptomless, certain high-risk strains can integrate into human DNA, triggering cancerous changes in cervical cells.
The introduction of the Gardasil vaccine in the early 2000s revolutionized prevention. Since then, cervical cancer rates have sharply declined in countries with long-standing vaccination programs. In the US, the vaccine is routinely given to girls and boys aged 9–14, building community immunity that protects both sexes.
Global progress and persistent challenges
Globally, the World Health Organization (WHO) aims to eliminate cervical cancer as a public health threat. Its strategy includes vaccinating 90% of girls by age 15, screening at least 70% of women twice in their lifetime, and ensuring 90% of diagnosed women receive timely treatment. WHO modeling suggests achieving these goals could prevent up to 74 million new cases of cervical cancer by 2100.
Evidence from Sweden supports this optimism. A study of more than 1.6 million women found half as many cervical cancer cases among those vaccinated compared to those who were not. Dr. Jiayao Lei, lead author and epidemiologist at the Karolinska Institute, emphasized that early vaccination — ideally before sexual activity begins — offers the strongest protection.
“Those who are vaccinated at an earlier age will have stronger protection,” said Dr. Lei.
Vaccine rollouts in developing countries
Developing nations are catching up. Pakistan has launched its HPV vaccination program targeting girls aged 9–14, while Ghana is set to begin distribution later this year. Despite misinformation campaigns — including false claims linking the vaccine to infertility — Pakistan’s Health Ministry reports that nine million girls have already been vaccinated, with a target of 13 million.
Maintaining the momentum
Public health experts stress that sustaining high vaccination rates is critical for maintaining herd immunity. With vaccine skepticism on the rise globally, even small declines in coverage could allow HPV infections to rebound.
“The vaccine is extremely effective, but only if it gets into arms,” Dr. Kahn warned.
As the US inches closer to cervical cancer herd immunity, the challenge lies not in science — but in public trust, access, and the continued commitment to vaccination.
