In a major development for breast cancer treatment, researchers in the UK have discovered that artificial progesterones, commonly used to alleviate hot flashes caused by anti-estrogen therapy, may also slow the growth of estrogen receptor-positive (ER-positive) breast cancer. This exciting finding, reported in the PIONEER trial, highlights the potential for improving both treatment adherence and tumor control in patients battling this common form of breast cancer.

The link between hot flashes and tumor growth

Breast cancer is the most common cancer among women worldwide, and around 75% of these cancers are estrogen receptor-positive (ER-positive), meaning they are fueled by the hormone estrogen. To treat ER-positive breast cancer, many women are prescribed anti-estrogen medication, such as letrozole, which blocks estrogen production in the body. However, these treatments often cause unpleasant side effects, including hot flashes, joint pain, and bone density loss, resembling the symptoms of menopause.

To manage these side effects, many women are prescribed artificial progesterones like megestrol acetate. Now, the PIONEER trial has revealed that megestrol acetate, even at a low dose, not only helps alleviate hot flashes but may also slow the growth of ER-positive breast tumors. This dual benefit could significantly improve patient outcomes and quality of life during treatment.

The PIONEER trial: Promising results

The PIONEER trial, conducted in the UK, involved 244 women with early-stage ER-positive breast cancer. Participants were divided into three groups: one receiving letrozole alone, another receiving letrozole plus a 40mg dose of megestrol acetate, and a third group receiving letrozole plus 160mg of megestrol acetate.

Researchers found that megestrol acetate enhanced the anti-proliferative (tumor-slowing) effect of letrozole. Notably, even the lower dose of megestrol was just as effective as the higher dose in slowing tumor growth. This breakthrough could help reduce the adverse effects of treatment while improving tumor control, potentially leading to better long-term outcomes for patients.

Benefits beyond hot flashes

While megestrol acetate has long been used to alleviate the hot flashes caused by anti-estrogen therapy, its potential to slow tumor growth is a new and exciting development. According to Dr. Richard Baird, the study’s corresponding author, the results suggest that megestrol acetate could provide a dual benefit: alleviating hot flashes to improve treatment adherence and directly inhibiting tumor growth through its impact on estrogen signaling.

"Reducing side effects is crucial for encouraging long-term adherence to treatment, and low-dose megestrol may offer a way to achieve this without sacrificing efficacy," Dr. Baird explained.

Caution: Side effects and further studies needed

Despite the promising results, megestrol acetate is not without its risks. Side effects like weight gain, high blood pressure, and the potential for blood clots must be considered, especially at higher doses. However, the PIONEER trial’s findings suggest that a lower dose could be just as effective in slowing tumor growth while minimizing these risks.

While these results are promising, they are based on a small study group, and the treatment duration was only two weeks. Larger, long-term studies are necessary to confirm these findings and establish definitive clinical guidelines for using megestrol acetate in breast cancer treatment.

Looking ahead: More research needed

As Dr. Esha Sachdev, a breast medical oncologist, pointed out, more trials are needed to validate these results and determine the best dosing regimen for megestrol acetate in combination with letrozole. "Larger, randomized phase 3 trials comparing letrozole plus megestrol versus letrozole alone are essential to establish a new clinical guideline," she said.
