SWITZERLAND: A luxury expedition voyage meant to explore some of the world’s most remote and breathtaking regions has suddenly turned into a high-level international public health emergency.
The World Health Organization (WHO) is now closely monitoring a deadly cluster of severe respiratory illness linked to a Dutch-flagged cruise ship carrying 147 passengers and crew members from 23 nationalities. As investigations intensify across multiple countries, the outbreak has triggered global concern over hantavirus—a rare but potentially fatal disease most people know little about.
So far, seven cases have been identified, including two laboratory-confirmed hantavirus infections, three deaths, one critically ill patient in intensive care, and several additional suspected infections still under investigation.
The situation has rapidly evolved into a coordinated international response involving Cabo Verde, the Netherlands, South Africa, Spain, the United Kingdom, Argentina, and WHO itself.
And while WHO currently assesses the risk to the global public as low, the seriousness of the illness—and the mystery surrounding how exposure occurred—has placed global health agencies on alert.
A voyage across remote regions turns into an international health investigation
The cruise ship departed Ushuaia, Argentina, on 1 April 2026 and traveled through ecologically sensitive and isolated areas across the South Atlantic, including mainland Antarctica, South Georgia, Nightingale Island, Tristan da Cunha, Saint Helena, and Ascension Island.
Authorities are now investigating whether passengers may have been exposed to infected rodents or contaminated environments either during shore excursions or before boarding in South America.
The vessel, now moored off the coast of Cabo Verde, carried 88 passengers and 59 crew members during the voyage.
What initially appeared to be isolated illnesses quickly escalated into a far more alarming situation.
How the outbreak unfolded
The first known patient—an adult male—developed fever, headache, and mild diarrhoea on 6 April while onboard. Within days, his condition deteriorated rapidly into severe respiratory distress, and he died aboard the ship on 11 April.
No laboratory testing was conducted before his death.
A second patient, an adult female who had close contact with the first case, later developed gastrointestinal symptoms while on Saint Helena. Her condition worsened dramatically during a flight to Johannesburg, South Africa. She died on 26 April after arrival at a hospital emergency department.

A third confirmed patient developed fever, pneumonia, and breathing difficulties before being medically evacuated from Ascension Island to South Africa, where he remains hospitalized in intensive care.
Additional suspected cases involving high fever and gastrointestinal symptoms remain under active evaluation.
Health teams are continuing laboratory investigations, including sequencing and metagenomic analysis, to better understand the outbreak.
What is hantavirus—and why are experts taking it seriously?
Hantavirus Pulmonary Syndrome is a severe zoonotic disease caused by hantaviruses carried primarily by rodents.
Humans typically become infected through contact with rodent urine, saliva, faeces, or contaminated surfaces. Infection can occur while cleaning contaminated spaces, handling infested materials, or entering poorly ventilated environments where viral particles may become airborne.
The disease is rare—but extremely dangerous.
Symptoms often begin with fever, headache, chills, muscle pain, nausea, vomiting, diarrhoea, and abdominal discomfort before rapidly progressing to pneumonia-like illness, respiratory failure, shock, and acute respiratory distress syndrome.
In severe cases, patients can deteriorate with alarming speed.
WHO notes that symptoms generally appear between one and eight weeks after exposure.
Why this outbreak is raising global concern
One reason international agencies are taking the event seriously is the unusual setting.
Cruise ships present unique public health challenges because of prolonged close contact among passengers, international travel routes, and shared enclosed environments.
Although hantavirus is usually linked to rural or wilderness exposure rather than ships themselves, the expedition itinerary included remote ecological regions where rodent exposure may have occurred.
Another critical concern is the possibility—however limited—of human-to-human transmission.
WHO emphasizes that most hantavirus infections do not spread easily between people. However, certain South American strains, particularly Andes virus, have previously shown rare human-to-human transmission in situations involving prolonged close contact.
That possibility is now part of ongoing investigations.
WHO launches coordinated international response
Health authorities across multiple countries have activated emergency coordination measures under the International Health Regulations framework.
WHO has initiated three-level coordination and is supporting:
- Epidemiological investigations
- Isolation and medical management of suspected cases
- International contact tracing
- Laboratory testing and genomic analysis
- Medical evacuation logistics
- Public health communication efforts
Argentina has already shared passenger and crew lists with relevant countries based on nationality to support follow-up and surveillance.
Passengers onboard have also been advised to remain in cabins where possible and practice strict physical distancing measures.
Why WHO says the global risk is still low
Despite the alarming deaths, WHO currently assesses the overall risk to the global population as low.
Several factors support this assessment:
Hantavirus infections remain relatively uncommon worldwide. Human transmission is rare, and most exposure events occur through direct environmental contact with infected rodents rather than casual person-to-person interaction.
WHO has not recommended any travel or trade restrictions related to the outbreak.
At the same time, the agency continues to stress vigilance, especially among travelers, healthcare workers, cruise operators, and people returning from areas where hantavirus circulation is known.
The hidden danger of rodent-borne diseases
The outbreak is also drawing attention to a broader global health issue often overlooked until crises emerge: zoonotic disease spillover.
Environmental disruption, climate variability, ecotourism, expanding human interaction with wildlife habitats, and changing ecosystems continue increasing opportunities for infectious diseases to cross from animals into humans.
Hantavirus outbreaks have historically been linked to fluctuations in rodent populations influenced by weather patterns and ecological changes.
In the Americas alone, WHO reports that eight countries documented 229 hantavirus cases and 59 deaths during 2025, reflecting a case fatality rate exceeding 25 percent.
Some strains in the Americas can carry mortality rates approaching 50 percent.
No cure exists—but early care can save lives
Currently, there is no approved antiviral cure or vaccine for hantavirus pulmonary syndrome.
Treatment depends heavily on early recognition and aggressive supportive care.
WHO advises that patients with suspected infection require rapid transfer to emergency or intensive care facilities where advanced respiratory support, mechanical ventilation, hemodynamic monitoring, and even extracorporeal oxygenation may become necessary.
Experts emphasize that delayed diagnosis can dramatically worsen survival chances.
What travelers and the public should know
WHO advises travelers and the public not to panic—but to remain informed.
The organization recommends:
- Frequent hand hygiene
- Avoiding rodent-infested environments
- Never dry-sweeping contaminated spaces
- Maintaining ventilation in enclosed areas
- Seeking medical attention early if symptoms develop
- Self-isolating when respiratory symptoms appear
The agency also stresses that most routine tourism activities remain very low risk.
A reminder of how quickly health threats can emerge
The cruise ship outbreak is a reminder that in an interconnected world, even rare diseases can rapidly trigger international concern.
One unexplained illness aboard a remote expedition vessel has now mobilized multiple governments, laboratories, emergency response teams, and global health authorities.
And while WHO says there is no evidence of a broader public threat at this stage, the incident underscores how fragile the balance between travel, ecology, and infectious disease can become.
For now, the world is watching closely as investigators race to determine exactly how this deadly outbreak began—and whether further cases may still emerge.
